A care consultation is a one-on-one supportive space for you to talk through a difficult healthcare situation and have a partner to help navigate what comes next.
Provided via online video or phone call, a 60-minute session allows our consultant to understand your unique situation and help you determine your next step in a variety of situations:

You are just starting to notice brain changes in yourself or someone else and are not sure what to do next.

There is a new diagnosis of Alzheimer's or another type of dementia and you would like to know what to expect.

You are experienced on the journey of dementia and now face a new challenge.

You or a loved one is hospitalized for an acute or chronic illness and you aren't sure what to do next for discharge planning.
This 90-minute session includes family, friends, and other supports to meet together via online video with our consultant and figure out “What do we do next?” Our consultant can help steer you through challenges, such as:

Navigating the healthcare system during or following hospitalization.

Determining the best role for each person in the support system.

Approaching new or ongoing situations in dementia care when several support people are involved.

Planning ahead for long-term care.

Decision-making around end-of-life considerations in acute or chronic illness.
Sometimes all you need is a quick phone call! Every busy person knows that a whole hour can be hard to come by, but a 30-minute session is more feasible to squeeze in on your lunch break. These shorter consultations are offered via phone or online video.

Faith Community Support
As our population ages, faith communities are no different. We look forward to supporting your faith-based organization in any or all of these ways:

Faith Communities Facing Dementia series: This interactive program is specifically developed to help faith communities learn how to better share faith traditions with people living with dementia in their midst. It can be adapted for in-person or online video meeting, as well as for different lengths of time. Whether your group wishes to have a short luncheon program or a five-part series over several weeks, the insights you gain will build your faith community’s competence in welcoming people living with dementia and their caregivers.

Worship adaptations: We will look together at your worship service, both programming and space, and provide ideas for modifications to best support people living with dementia and their caregivers.

Congregational care: When members and clergy are preparing to make a home visit, we will provide individualized recommendations to help nurture spirituality and share faith traditions in ways that are more accessible to the person being visited.

This program can be tailored in length to anywhere between two and eight hours, is provided in-person or virtually, and is adapted to the specific scenarios your group encounters. Let us know what your group is interested in learning!
No matter how many people join us in-person or online, our speaker will engage the group to try new skills, learn best practices in communication, and demystify common challenges in dementia care. This program is adaptable to different lengths of time and a diversity of audiences.
